Cloud file servers and issues with access and permissions

In the last 5 days, a Cloud Enterprise services and configuration changes caused some issues for one of my major clients. Why? It was a permissions issue and eventually found it was a scope and root configuration issue. The havoc started out minor the first day, and then escalated after 2, 3, 4, and the 5th day it appeared more problematic. What do you do to fix it, look at the methods and steps used to sandbox and test the changes prior to and then schedule the maintenance even if an emergency window is required after hours for key people to test and confirm changes.
Over the last 30 years of doing systems administration, network management, server management, file management, email management, and when minor problems become major problems; it usually is a permissions problem; I could cite hundreds of examples, and the only solution is to make sure the permissions are properly reset and propagated quickly. Plus, look at sensitive folders and content that should be moved out of the public share tree and branches and make sure they are isolated so when a folder permissions and sub-folders and files needs to be reset, it does not leave anything wide open.
